Passports

PASSPORT BOOK OR PASSPORT CARD
Passport applications are accepted Monday through Friday from 9:00am to 3:30pm, except for legal holidays – BY APPOINTMENT ONLY. Please contact

Passport Book – can be used for land, sea or air travel

Applying for new passport book
To apply for a new passport, you will need to provide:

  • Completed DS-11 form, do not sign
  • One passport photo
  • An original or certified birth certificate (or other proof of citizenship)
  • An acceptable form of identification.
  • All applicants must be present, including children.
  • For children 15 years or younger, both parents must be present and provide identification

Fees
Standard Application (DS-11) – Check or money order only, payable to Department of State

  • $110 for those 16 years and older (one check or money order per applicant)
  • $80 for those 15 years and younger (one check or money order per applicant)

 

  • Facility Processing Fee – $25 per applicant (can be paid in a lump sum for all applicants)
    • Payable by cash, check, money order, credit card ($2 additional fee) or debit card ($1 additional fee).

Passport Card – can only be used for land travel to Canada and Mexico

Applying for a new passport card
To apply for a new passport card, you will need to provide:

  • One passport photo
  • An original or certified birth certificate (or other proof of citizenship)
  • An acceptable form of identification.
  • All applicants must be present, including children
  • For children 15 years or younger, both parents must be present and provide identification

You may obtain applications for new passports, renewals or amendments to a passport at City Hall or by visiting http://travel.state.gov/passport/get/get_4855.html.

Your birth certificate will be returned to you by the Federal Passport Agency, along with your passport, in approximately 4-6 weeks. If you have any questions, please call City Hall at (360) 893-2219 ext. 113 or email jhelgelien@cityoforting.org.

Birth Certificates
Birth certificates for passport purposes can be ordered online at www.vitalcheck.com (except for California birth certificates). When ordering be sure to state “for passport purposes.” Birth certificates must show full name of parents.

Children
It is required to have both parents present with an applicant 15 years or younger as provided by Public Law 106-113, Section 236. If only one parent can appear, that parent must present evidence of sole legal custody or have the absent parent complete Form DS-3053, Statement of Consent, which must be notarized and submitted with the child’s application.

Eligible Proof of Sole Legal Custody:

  • Child’s certified U.S. or foreign birth certificate listing only applying parent.
  • Report of Birth Abroad (FS-240) or Certificate of Birth Abroad (DS-1350) listing only one parent o Court order granting sole custody (unless child’s travel is restricted by that order)
  • Adoption decree (listing only the applying parent)
  • Court order specifically permitting applying parent’s or guardian’s travel with the child
  • Judicial declaration of incompetence of non-applying parent
  • Death certificate of non-applying parent
  • Written consent of the other parent for the issuance of the passport
  • Written statement explaining why non-=apply parent’s consent cannot be obtained

If no parent is available to sign/appear, the third party in loco parentis (taking on responsibilities of a parent for someone else’s child) must appear with a notarized written statement or affidavit from both parents/custodial parent(s) authorizing the third party to apply for a passport. When the notarized statement or affidavit is from only one parent, the third party must present evidence of sole custody of the authorizing parent.

Passport Renewal
Applicant completes and mails to passport agency. This form can only be used for passports issued within the last 15 years, if application was at least 16 years old when issued, and if the applicant can appropriately answer the four questions listed on the application.

Expedited Service
An additional $60 is charged by the Department of State for expedited service, which provides processing within a two week period. If processed by facility, an additional $12.85 overnight/certified postage fee is charged.
